This notice covers patient information maintained by HOWDY IV LLC, the mobile IV therapy practice serving College Station, Texas. It explains our privacy practices and the records and complaint procedures described below. The website privacy page separately describes information associated with browsing this website.
Privacy and records contact
Contact HOWDY IV LLC at privacy@howdyiv.com about this notice, a records request, or a privacy concern. Please ask for secure instructions before sending medical information or identification documents. Do not send those details to our general email inbox.
Patient records and electronic disclosures
Patient charts, including Good Faith Exam documentation and treatment orders, are maintained in Aesthetic Record. Appropriate professionals and staff use relevant information to evaluate and provide care, coordinate visits, prepare bills, and administer the practice.
Your health information may be disclosed electronically. Applicable law may permit disclosures for treatment, payment, health care operations, or other authorized or required purposes. We obtain your authorization when the applicable law requires it. A notice acknowledgment is not permission to disclose your records for unrelated purposes.
We consider the legal basis and any limits before responding to requests from third parties, including legal process. A request or subpoena does not automatically authorize disclosure. Additional protections may apply to information received from another provider or to specially protected records.
How to request your health care records
- Email privacy@howdyiv.com with the subject “Health records request” and ask for instructions for submitting a written request securely. Include a way we can contact you; do not attach records, identification, or medical details to the initial email.
- In your written request, identify the records and dates you want, your preferred format and delivery method, and whether you are requesting your own records or acting for someone else.
- Follow the secure identity-verification instructions we provide. A representative must establish their authority to request the records.
We act on requests within the applicable legal timeframe. When Texas electronic-record law applies and the electronic system can fulfill your written request, the requested electronic record must generally be provided by the 15th business day after receipt, unless you agree to another form and subject to lawful access exceptions. We follow any shorter applicable deadline.
Any records fee must be permitted by law. We explain any applicable fee and any lawful limit on access. If you believe a record is incorrect or need a different communication method, contact us at the privacy email above so we can address your request under the applicable requirements.
Privacy during home visits
Tell your clinician if you would like a private discussion or have a communication preference. We take reasonable steps to protect privacy during a home visit. Use the clinical communication instructions provided by your care team for health questions.
Concerns and complaints
You may raise a privacy or records concern with HOWDY IV LLC at privacy@howdyiv.com. You may also contact the appropriate regulator. We do not retaliate against you for raising a concern or filing a complaint.

Safeguards, notices, and copies
We protect patient information under applicable law and provide any legally required breach notice. A privacy concern or technical incident does not necessarily mean a reportable breach has occurred.
